Hello Everyone,
Please let me know if you have a youth who is in DCF Care and Placement who is a senior and who will be graduating this year from High School. They are able to get $300 towards senior expenses. Please fill out the attached life skills request along with a letter from the youth stating who they are, what they will be using the money towards (i.e. prom, school pictures etc) with their signature, and a signed w-9 form (also attached).
