If you have a youth in DCF care and placement who needs assistance to purchase a computer for school, please complete the attached Life Skills Form and W-9, along with a request letter from the teen. Youth may request $650 for a laptop, printer and accessories.

 

We are also submitting senior expense requests for youth graduating June 2014.

 

Please give the completed forms to your Outreach Worker.
