Hi Everyone,
Please let me know if you have a youth who is in DCF Care and Placement who is a senior and who will be graduating this year from High School (2014). They are able to get $300 towards senior expenses. Please fill out the attached life skills request along with a letter from the youth stating who they are, what they will be using the money towards (ie prom, school pictures, class trips etc) with their signature, and a signed w-9 form (also attached). Please ensure that there is an open mailing address in family net that will match the address on the signed w-9. Let me know if you have any questions.
